Home Care Packages
Evergreen Care is an approved provider of government-funded Home Care Packages. These packages are designed to provide a coordinated package of care and services to help you live independently in your own home. There are four levels of Home Care Packages, from basic support (Level 1) to high-level care needs (Level 4).
We can work with you to develop a personalised care plan that makes the most of your package funding and meets your specific needs and goals.
Getting Started
To access our Home Care services, you will first need to be assessed by the government's My Aged Care service. They will determine your eligibility for services and the level of support you require.
